Registered Nurse (RN) – Home Health Case Manager

**$5,000 Sign-On Bonus | Full-Time & PRN | $65,000–$90,000/year**

### What You’ll Do

As a **Home Health Case Manager RN**, you will provide skilled nursing care while coordinating a patient's overall plan of care.

Your responsibilities will include:

-   Manage an assigned **home health nursing caseload**
-   Provide skilled nursing visits, evaluations and starts of care
-   Supervise LPN visits and coordinate care among healthcare providers
-   Complete comprehensive nursing assessments and develop individualized plans of care
-   Communicate with physicians to obtain orders and establish diagnosis-driven plans of care
-   Coordinate with insurance companies and other healthcare professionals
-   Educate patients and families about conditions including **CHF, COPD, diabetes, wound care and medication management**
-   Collaborate with **Physical Therapy, Occupational Therapy and Speech Therapy**
-   Provide hands-on patient care in patients' homes
-   Use clinical judgment, physical assessment skills and diagnostic information to support quality patient outcomes

### What We’re Looking For

-   **Missouri RN license or Compact RN license**
-   Valid **driver’s license**
-   Reliable vehicle with current auto insurance
-   **Home health nursing experience preferred**
-   **Critical care experience preferred**
-   Strong assessment, communication and clinical decision-making skills
-   Ability to work independently while collaborating with an interdisciplinary care team
