**Position Overview:**  

Greenlife Healthcare Staffing is currently seeking a Licensed Practical Nurse (LPN) to fill openings with a skilled care facility located in Queens, New York. This per-diem position in long-term care is ideal for a dedicated LPN with nursing home or hospital experience who is passionate about providing quality care to residents in a skilled care setting.

## Requirements

**Qualifications:**  

-   **Education:** Graduation from an accredited Practical Nursing program.
-   **Licensure:** Must have an active New York State Licensed Practical Nurse (LPN) license. LPN Registration required.
-   **Experience:** Must have at least two (2) years of experience in a nursing home or hospital setting.
-   **Technical Skills:** Proficiency in medication administration, wound care, vital signs monitoring, and documentation using electronic or paper systems.
-   **Required Documentation:**

-   Driver's License or Passport or Green Card and SSN Card
-   COVID Vaccination Card
-   MMRV Results (Mumps, Measles, Rubella, and Varicella)
-   Physical Assessment
-   CPR Certification

**Key Responsibilities:**  

-   Administer medications and treatments as prescribed.
-   Monitor and document patients' vital signs and health status.
-   Collaborate with RNs, physicians, and therapists to implement care plans.
-   Assist residents with activities of daily living (ADLs) and mobility.
-   Ensure compliance with infection control and facility protocols.
-   Respond to emergencies and provide timely interventions.
-   Maintain accurate and timely medical records.
