Telehealth Physician (Remote | Full-Time | No On-Call) Monday- Friday 9am-6pm

### What You’ll Do

-   Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
-   Create and manage personalized care plans for ongoing conditions
-   Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
-   Ensure thorough documentation in our EHR and compliance with quality measures
-   Engage patients in preventive care to reduce avoidable complications

## Requirements

### What We’re Looking For

-   MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
-   1+ year of physician experience in family medicine or internal medicine
-   Active medical license — multi-state licensure a plus
-   Experience in preventive care, senior health, or chronic condition management is a plus
-   Comfortable practicing via telehealth and using EHR systems is a plus
-   Experience providing preventive virtual care is a plus
