## Description

**Job Title:** Family Medicine Advanced Practice Professional (Nurse Practitioner / Physician Assistant)

**Position Summary:**  
We are seeking a skilled Family Medicine Advanced Practice Professional (NP or PA) to provide comprehensive, patient-centered primary care across the lifespan. The APP will evaluate, diagnose, and manage acute and chronic conditions, provide preventive care, and collaborate with healthcare teams to optimize patient health outcomes.

**Key Responsibilities:**

-   Conduct patient histories, physical examinations, and diagnostic assessments.
-   Evaluate, diagnose, and manage acute and chronic illnesses across all age groups.
-   Develop and implement individualized care plans, including medication management and referrals to specialists when needed.
-   Provide preventive care, health maintenance counseling, and patient education on lifestyle, nutrition, and disease prevention.
-   Perform procedures within scope of practice, such as immunizations, minor wound care, and screenings.
-   Document patient encounters accurately and maintain compliance with regulatory and institutional standards.
-   Collaborate with physicians, nursing staff, and other healthcare professionals to ensure coordinated, high-quality care.
-   Participate in quality improvement initiatives, continuing education, and professional development.

**Qualifications:**

-   Master’s degree in Nursing (NP) or completion of an accredited Physician Assistant program.
-   Active NP or PA licensure in the state of employment.
-   National certification as an NP or PA.
-   Experience in family medicine or primary care preferred.
-   Strong clinical assessment, decision-making, and communication skills.
