We are seeking an experienced Nurse Supervisor to lead our Care Management team.

Your proficiency in discharge planning and utilization review will be highly valued as you guide our department to achieve exceptional patient care outcomes.

## Requirements

-   This candidate will provide Associate Director level supervision to the Care Management Department
-   Must have 2 years of experience in discharge planning and utilization review
-   Must be CCM & PRI certified
-   Bachelor’s degree required but masters preferred
