### About the Role

The **Clinical Case Manager** supports clients throughout their recovery by assessing needs, coordinating access to internal and external resources, and promoting long-term success. As a member of the interdisciplinary team, this position collaborates effectively with colleagues while independently managing priorities, addressing challenges, and advocating for client needs.

The Clinical Case Manager provides individual and group clinical services within the scope of their credentials and appropriate clinical supervision, while maintaining or working toward **Certified Recovery Support Worker (CRSW)** or **Licensed Alcohol and Drug Counselor (LADC)** certification/licensure.

This role also contributes to the safety, stability, and daily operations of Hope on Haven Hill's treatment programs. Through ongoing assessment, collaboration, and continuous improvement, the Clinical Case Manager helps create a recovery-oriented environment and supports high-quality outcomes for clients and their families.

## Requirements

### Minimum Position Requirements

**Required:**

-   Current certification in New Hampshire as a **Certified Recovery Support Worker (CRSW)**, or willingness to obtain CRSW certification within 12 months of employment.
-   High school diploma or equivalent.
-   Must be at least 18 years of age.
-   Strong problem-solving skills, with the ability to analyze issues and implement timely, effective solutions.
-   Exceptional communication and interpersonal skills, with the ability to engage clients, families, and colleagues respectfully and professionally.
-   Strong written communication, documentation, and computer skills.
-   Ability to manage multiple priorities efficiently while maintaining attention to detail.
-   Demonstrated commitment to excellence in client service and collaborative teamwork.
-   Ability to provide constructive input into program development, systems improvement, and process efficiencies.
-   Strict adherence to confidentiality standards, including compliance with **42 C.F.R. Part 2 and HIPAA regulations**.

**Preferred:**

-   Bachelor's degree in human services, social work, counseling, psychology, or a related field, or equivalent relevant experience.
-   Current New Hampshire **Licensed Alcohol and Drug Counselor (LADC)** credential.
-   Candidates who are not currently licensed as an LADC must be willing to participate in appropriate clinical supervision and pursue LADC licensure.
-   Experience working with individuals with substance use disorders, co-occurring behavioral health needs, or other complex social-service needs.
-   Experience providing case management, recovery support, counseling, or other direct client services.
-   Experience working with pregnant and parenting women, children, or families.
-   Knowledge of trauma-informed care, recovery-oriented systems of care, and community-based behavioral health resources.
