Are you a compassionate Physical Therapist who values personalized care, independence, and flexibility? Join our supportive Home Health team in Greenwich, Connecticut, where you’ll have the opportunity to provide one-on-one therapy that helps patients regain strength, mobility, confidence, and independence—all from the comfort of their own homes.

This is an excellent opportunity for a PT who enjoys building meaningful patient relationships while having the flexibility to manage their own schedule and maintain a healthy work-life balance.

### What You’ll Do

-   Provide high-quality, patient-centered physical therapy services in patients’ homes.
-   Evaluate patients’ mobility, strength, functional abilities, and rehabilitation needs.
-   Develop and implement individualized treatment plans focused on improving independence and quality of life.
-   Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
-   Educate patients and families on exercises, therapy techniques, safety, and lifestyle modifications.
-   Collaborate with nurses, physicians, therapists, and other healthcare professionals to provide coordinated care.
-   Maintain accurate and timely documentation of patient visits and progress.

## Requirements

-   Graduate of an accredited Physical Therapy program.
-   Current Connecticut Physical Therapist license.
-   At least 1 year of experience as a practicing Physical Therapist.
-   Home Health Physical Therapy experience preferred.
-   Strong communication and interpersonal skills with a compassionate approach to patient care.
-   Ability to work independently, manage priorities, and organize a flexible patient schedule.
-   A genuine passion for helping patients achieve greater mobility and independence.
