## Description

**LPN - Long Term Care - Pay Rate: $35.00+ per hour - Evening Shifts**

**WorkFit Medical Staffing** is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team in providing exceptional care in long-term care facilities. **Position Overview:**

-   Deliver professional nursing care to residents in long-term care settings.
-   Administer medications and treatments as prescribed by physicians.
-   Monitor and assess residents’ health status and report any changes promptly.
-   Collaborate with healthcare team members to develop and implement individualized care plans.
-   Maintain accurate documentation of resident care and health information.
-   Ensure compliance with facility policies, procedures, and regulatory standards.

## Requirements

-   Current and valid NY State LPN license.
-   Minimum of 1 year experience in long-term care nursing preferred.
-   Basic Life Support (BLS) certification.
-   Strong clinical skills and attention to detail.
-   Effective communication and interpersonal skills.
-   Ability to work collaboratively within a multidisciplinary healthcare team.
-   Compassionate and patient-centered approach to care delivery.
