At H2 Health Dublin, you'll work with a diverse patient population that includes orthopedic conditions, post-surgical rehabilitation, sports-related injuries, balance and mobility concerns, and functional movement impairments across pediatric, adult, and geriatric populations. **Your Role**

**Physical Therapist (PT)**

-   Evaluate and develop individualized treatment plans for patients with orthopedic and musculoskeletal conditions.
-   Deliver evidence-based physical therapy interventions to help patients achieve their functional goals.
-   Collaborate with physicians, referral sources, patients, and caregivers to ensure coordinated care.
-   Monitor patient progress and modify treatment plans as appropriate.
-   Maintain timely and accurate documentation using technology designed to streamline clinician workflows.

**Physical Therapist Assistant (PTA)**

-   Implement treatment plans established by the supervising Physical Therapist.
-   Provide therapeutic exercises, manual techniques, functional training, and patient education.
-   Monitor patient response to treatment and communicate progress to the Physical Therapist.
-   Assist patients in achieving improved mobility, strength, function, and independence.
-   Maintain accurate documentation and support an exceptional patient experience.

## Requirements

-   **Physical Therapist (PT)**

-   Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
-   Current Virginia Physical Therapist license or eligibility for licensure.
-   Strong communication, interpersonal, and clinical decision-making skills.
-   Passion for delivering patient-centered outpatient rehabilitation services.

-   **Physical Therapist Assistant (PTA)**

-   Associate degree from a CAPTE-accredited Physical Therapist Assistant program.
-   Current Virginia Physical Therapist Assistant license or eligibility for licensure.
-   Strong communication and teamwork skills.
-   Commitment to providing exceptional patient-centered care.
