**Job Summary**

Summit Healthcare Staffing is hiring on behalf of a client who is seeking a compassionate and detail-oriented **Bilingual Registered Nurse (RN)** to join their Intake team at a growing home health organization in **Brooklyn, NY**. This is an excellent opportunity for an experienced nurse who enjoys coordinating patient care, collaborating with interdisciplinary teams, and ensuring patients receive a seamless start to home health services.

**This position begins on-site in Brooklyn, with the opportunity to transition into a remote role based on performance and training.**

**Responsibilities**

-   Review and process incoming home health referrals.
-   Verify patient eligibility, insurance coverage, and required authorizations.
-   Coordinate timely admissions to home health services.
-   Ensure referrals meet clinical and regulatory requirements.
-   Communicate with referral sources, physicians, clinicians, and patients to facilitate a smooth admission process.
-   Collaborate with the Billing and Authorization teams to ensure accurate documentation.
-   Maintain compliance with agency policies and state and federal regulations.

**Compensation- $100,000-$120,000**

## Requirements

**Qualifications**

-   Active **Registered Nurse (RN)** license required.
-   Bilingual and able to communicate in Spanish
-   Previous experience in home health, intake, case management, or care coordination preferred.
-   Strong organizational and communication skills.
-   Ability to multitask in a fast-paced environment.
-   Experience with insurance verification and authorizations is a plus.
-   Detail-oriented with excellent clinical judgment.
