**Physical Therapist (PT) | Full-Time, Part-Time or PRN | Outpatient | 193 Glades Road, Berea, KY 40403**

At H2 Health @ Berea, you’ll work in a 2,500-square-foot outpatient clinic alongside an experienced therapy team with more than 80 years of combined experience. The clinic serves a diverse general outpatient population and offers programs across orthopedic and musculoskeletal rehabilitation, balance and dizziness, sports medicine, chronic pain, neurological conditions, manual therapy, Workers’ Compensation, and other rehabilitation needs.

**Your Role**

-   Evaluate patients and develop individualized, evidence-based Physical Therapy plans of care
-   Treat a diverse outpatient caseload focused on mobility, strength, function, and return to activity
-   Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training
-   Educate patients on movement, recovery, injury prevention, and home exercise programs
-   Collaborate with clinicians and support staff to coordinate high-quality care
-   Track patient progress and adjust treatment plans to support meaningful outcomes
-   Complete timely and accurate clinical documentation

## Requirements

-   Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program
-   Current Kentucky Physical Therapist license or eligibility for Kentucky licensure
-   CPR certification, if applicable
-   Strong communication and collaboration skills
-   Commitment to high-quality, patient-centered care
-   **New graduates are welcome to apply; previous outpatient experience is not required**
