## Description

RN Hospice Case Manager

_**(Full-Time, Permanent Hire Position with Excellent Full Benefits, Competitive Compensation/Bonus Structure and much more!)**_

**Position Responsibilities:**

·        Coordinate and oversee all aspects of assigned patients' hospice care

·        Develop and implement individualized plans of care with the interdisciplinary team

·        Perform head-to-toe assessments, monitor symptoms, and manage medications

·        Educate patients and families regarding condition, needs, and care

·        Communicate patient progress with clinical leadership, physicians, and the care team

## Requirements

·        _**Current Registered Nurse license.**_

·        Current driver's license

·        Current CPR certification

·        Two years of Registered Nurse experience in a clinical care setting

_**·        One year of full-time hospice experience**_

·        Reliable transportation and valid automobile liability insurance
