## Description

### Overview

Heritage Home Healthcare is seeking compassionate and dependable caregivers to provide in-home personal care services for Military Veterans in the Rio Rancho and Bernalillo areas. This PRN/on-call position is ideal for someone who is reliable, patient, and passionate about helping others maintain independence and dignity in the comfort of their homes.

Rio Rancho / Bernalillo Area

### Responsibilities

-   Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and meal assistance
-   Provide mobility and transfer assistance
-   Follow individualized care plans and delegated care instructions
-   Assist with light housekeeping and maintaining a safe home environment
-   Provide companionship and emotional support
-   Observe and report changes in client condition to the clinical team
-   Escort or transport clients to appointments and errands as needed
-   Maintain professionalism, confidentiality, and compassionate care at all times

### Qualifications

-   Must be at least 18 years old
-   Previous caregiving, CNA, home care, or healthcare experience preferred
-   Ability to work independently and follow care plans accurately
-   Strong communication and interpersonal skills
-   Valid driver’s license and reliable transportation required
-   Must live in Albuquerque, Rio Rancho, Bernalillo, or surrounding areas
