## Description

Estelle's Professional Community Services is seeking a dedicated and compassionate Qualified Mental Health Professional (QMHP) to join our Assertive Community Treatment (ACT) program team. ### Responsibilities

-   Conduct comprehensive mental health assessments to determine client needs and eligibility for services.
-   Develop and implement individualized treatment plans in collaboration with clients and interdisciplinary team members.
-   Provide crisis intervention and support to clients in community settings to ensure safety and stability.
-   Collaborate with psychiatrists, nurses, social workers, and other team members to coordinate care.
-   Monitor client progress and adjust treatment plans as necessary to achieve desired outcomes.
-   Facilitate client access to community resources such as housing, employment, and healthcare services.
-   Maintain detailed documentation and comply with all regulatory and agency standards.

## Requirements

-   Hold a valid QMHP certification or equivalent licensure as required by state regulations.
-   Minimum of a bachelor’s degree in psychology, social work, counseling, or a related field.
-   Experience working with individuals diagnosed with serious mental illnesses within community-based programs.
-   Strong knowledge of ACT model principles and evidence-based mental health practices.
-   Excellent communication and interpersonal skills to effectively engage clients and team members.
-   Ability to work independently and manage multiple cases with flexibility and professionalism.
-   Willingness to provide services in community settings, including client homes and other locations as needed.
