## Description

Genesis PHCM is a physician-led healthcare platform built to expand access to care through a modern, patient-centered model. Our foundation is virtual-first, providing 24/7 telehealth services to patients across the United States, while expanding our services onsite through urgent care and a Primary Care Clinic.

We are seeking an organized, proactive, and patient-focused Patients Care Coordinator to help turn prospective patient interest into scheduled appointments and make accessing Genesis’s services as simple as possible.

The ideal candidate is comfortable working independently, building processes from the ground up, and contributing ideas and best practices within a growing healthcare organization.

This role will report to the Executive Operations Lead and will manage patient follow-up, scheduling, and lead tracking through existing electronic medical record (EMR) systems. They will also support patient recruitment and assigned healthcare partnership outreach, helping ensure every prospective patient has a clear next step.

- Manage incoming patient inquiries and assigned leads from healthcare fairs, referral partners, and digital campaigns, completing timely follow-ups and documenting outcomes
- Guide prospective patients from initial interest through scheduling, answer non-clinical questions, explain appointment procedures using approved service information and pricing, and route clinical questions to the appropriate provider
- Monitor incoming communications, outreach history, prospect engagement levels, and pending outreach tasks while handling booking, verification, and calendar modifications with precision
- Support patient recruitment within assigned markets and audiences, including busy professionals, remote workers, frequent travelers, small businesses, and senior communities
- Support healthcare partnership outreach by researching organizations, identifying appropriate contacts, arranging discussions, and completing assigned follow-ups under the Executive Operations Lead’s direction
- Help establish patient lead and scheduling workflows, document procedures, and identify recurring barriers that prevent interested patients from booking appointments
- Conduct routine audits of patient charts to identify documentation gaps, ensure clinical accuracy, and support optimal billing workflow

## Performance Expectations (Key Results)

Within this role, expected outcomes include:

- Establish a clear patient lead workflow from initial inquiry through contact, follow-up, scheduling, and appointment
- Maintain accurate records so every assigned lead has a documented status, outreach outcome, and next step
- Use EMR system confidently to manage inquiries and complete accurate patient scheduling
- Establish consistent follow-up procedures for leads from events, partnerships, and digital campaigns
- Track response times, follow-up completion, appointment bookings, and lead-to-appointment conversion, and recommend improvements

## Requirements

### **Minimum Requirements**

- Experience working in a remote startup or similarly fast-paced, evolving environment
- Availability to provide approximately 30 hours of support per week
- Availability for company-wide meetings and collaboration, when needed, between 10:00 AM and 12:30 PM Pacific Time
- Experience in patient coordination, medical scheduling, healthcare administration, patient outreach, or a related patient-facing role
- Experience following up with prospective patients, managing inquiries, and helping individuals complete appointment scheduling or other next steps
- Experience using an electronic medical record (EMR), practice management platform, CRM, or similar system to maintain accurate records and track follow-ups
- Strong communication, follow-through, organizational, and recordkeeping skills, with a warm and professional approach to patient interactions
- Ability to explain non-clinical information clearly, recognize questions requiring clinical input, and handle confidential patient information appropriately in compliance with HIPAA guidelines
- Ability to work independently, manage multiple priorities, and coordinate effectively with internal team members and external contacts

### **Preferred Requirements**

- Experience using PatientPop and Tebra
- Experience supporting telehealth services or a primary care practice

- Full fluency in Spanish, both spoken and written
- Interest in building a long-term relationship with Genesis as the organization grows

## Benefits

- Approximately **30 hours per week**
- **Flexible working hours** outside of required meetings and collaboration needs
- Fully remote engagement
- Potential opportunity to transition from a freelance engagement to **W-2 employment** as Genesis grows, based on business needs and mutual fit
