## Description

**Licensed Practical Nurse (LPN) – Per Diem Night Shift Hospice**

We are seeking experienced **Licensed Practical Nurses (LPNs)** for **per diem night shift hospice assignments** throughout the **Fort Lauderdale to Boca Raton area**.

In this role, you will provide compassionate, high-quality care to patients in their homes in accordance with prescribed medical treatment, agency policies, and individual plans of care. LPNs may perform designated nursing procedures under the supervision of a Registered Nurse.

Pay: $27-29

Shift: 8hrs

**Responsibilities:**

-   Provide skilled nursing care in patients' homes
-   Follow established plans of care and physician orders
-   Monitor and report patient conditions and changes
-   Maintain accurate documentation
-   Communicate effectively with patients, families, and healthcare team members
-   Perform nursing procedures as authorized under RN supervision

## Requirements

-   Active Florida LPN license

-   **Strong Hospice experience required**

-   Experience with **HCHB (Homecare Homebase)** required

-   Reliable transportation

-   All onboarding requirements

-   Required online learning modules

-   Mandatory **8-hour in-person training**
