-   **Schedule:** Monday–Friday | 8:00 AM–5:00 PM
-   **Hutchinson, KS and surrounding areas**

**Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones.** Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting end-of-life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

**Requirements**

-   **Master’s in Social Work (MSW) or LCSW required**
-   Hospice experience preferred
-   Strong communication and interpersonal skills
-   Ability to work independently and as part of a team
-   Professional, dependable, and organized

**Responsibilities**

-   Provide counseling and support to patients and families in the home
-   Assist with advance directives and community resources
-   Participate in the hospice interdisciplinary team (IDG)
-   Coordinate referrals and additional support services
-   Maintain positive relationships with patients, families, staff, and referral partners
