Registered Nurse (RN) – Home Care

### Job Summary

Eminence Home Care is seeking a compassionate and dedicated **Registered Nurse (RN)** to join our growing home care team. The RN will be responsible for conducting patient assessments, developing individualized care plans, supervising caregivers, and ensuring high-quality care is delivered in accordance with state regulations and company standards.

### Key Responsibilities

-   Conduct initial and ongoing patient assessments in the home.
-   Develop, implement, and update individualized patient care plans.
-   Perform supervisory visits to evaluate caregiver performance and patient satisfaction.
-   Monitor patients' health conditions and identify changes in care needs.
-   Provide education and guidance to patients, families, and caregivers.
-   Complete accurate and timely clinical documentation.
-   Ensure compliance with state regulations, agency policies, and healthcare standards.
-   Collaborate with caregivers, families, physicians, and office staff to coordinate patient care.
-   Assist with caregiver training, competency evaluations, and clinical support as needed.
-   Address patient concerns and provide recommendations to improve quality of care.

### Qualifications

-   Active and unrestricted Registered Nurse (RN) license in the applicable state.
-   Minimum of 1 year of nursing experience preferred.
-   Previous home care or home health experience preferred.
-   Strong assessment, communication, and organizational skills.
-   Ability to work independently and manage multiple patient cases.
-   Valid driver's license and reliable transportation.
-   Proficiency in electronic medical records (EMR) and clinical documentation.
