### RN/RPN – Complex Home & Community Care

**Brampton, Ontario**

The Care Company is looking for compassionate, dependable, and self-driven Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to support clients with complex medical needs throughout Brampton.

Our nurses provide one-to-one nursing care in home and community settings, supporting clients with varying levels of medical complexity. We are looking for nurses who are interested in developing consistent, ongoing client relationships and providing high-quality, person-centred care.

Previous complex-care, pediatric, or home and community nursing experience is a strong asset. We are happy to provide additional client-specific training to the right candidates.

### Scheduling

-   Opportunities are available for day, evening, overnight, weekday, and weekend shifts based on current client needs.
-   Both part-time and opportunities for full-time hours may be available.
-   We are seeking nurses who can commit to a consistent, ongoing weekly schedule.
-   Continuity of care is extremely important to our clients and families, and we prioritize nurses who are interested in maintaining long-term client assignments.
-   Overnight assignments are awake and alert shifts unless otherwise specifically indicated.

## Requirements

### Duties & Responsibilities

-   Provide safe, compassionate, one-to-one nursing care in accordance with individualized care plans and medical orders.
-   Complete ongoing nursing assessments and monitor clients for changes in respiratory, neurological, physical, cognitive, and overall clinical status.
-   Provide complex nursing interventions within your scope of practice and demonstrated competency.
-   Administer medications and treatments in accordance with medical orders, client care plans, and CNO standards.
-   Recognize and respond appropriately to changes in client condition and promptly escalate clinical concerns when required.
-   Support clients with activities of daily living, personal care, mobility, transfers, and other needs outlined in their individualized care plans.
-   Safely operate client-specific medical and mobility equipment in accordance with training and established protocols.
-   Maintain a strong commitment to infection prevention and control, client safety, professional standards, and person-centred care.
-   Collaborate with clients, families, Care Coordinators, physicians, Ontario Health atHome, and other members of the interdisciplinary healthcare team.
-   Complete accurate, thorough, and timely clinical documentation.
-   Practice safely and competently within your individual knowledge, skill, judgment, training, and professional scope of practice.

### Complex Care Experience

Previous hands-on experience with one or more of the following is a strong asset:

-   Tracheostomy care
-   Oral, nasal, and tracheal suctioning
-   G-tube and GJ/J-tube feeding and care
-   Ventilator support
-   BiPAP or CPAP
-   Cough assist
-   Oxygen therapy and respiratory monitoring
-   Seizure monitoring and management

Nurses with previous pediatric, complex-care, or home and community nursing experience are strongly encouraged to apply.

### Requirements

-   Current registration and good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
-   Negative TB test completed within the past 12 months.
-   Valid Criminal Record Check with Vulnerable Sector Screening completed within the past 12 months.
-   Valid CPR/First Aid certification.
-   Valid WHMIS 2015 certification completed within the past 12 months.
-   COVID-19 vaccination documentation, subject to accommodation in accordance with the Ontario Human Rights Code.
-   Ability to communicate effectively in English, both verbally and in writing.
-   Strong nursing assessment, clinical judgment, and critical-thinking skills.
-   Ability to identify, respond to, and appropriately report changes in client condition.
-   Ability to work independently in the community while collaborating effectively with the broader care team.
-   A valid driver's licence and access to a vehicle is an asset but not required.
