Commonwealth Medical Services is seeking a compassionate and dedicated **Family Medicine Physician** to join our primary care team in Lima, Ohio. This position offers the opportunity to provide comprehensive outpatient care for patients of all ages while working within a collaborative healthcare environment focused on preventive medicine, chronic disease management, and continuity of care. **Responsibilities**

-   Provide comprehensive primary care services for pediatric, adult, and geriatric patients.
-   Diagnose and treat acute and chronic medical conditions.
-   Perform preventive health screenings and annual wellness examinations.
-   Develop individualized treatment plans and coordinate specialty referrals when appropriate.
-   Educate patients regarding disease prevention, medications, nutrition, and healthy lifestyles.
-   Maintain accurate and timely medical documentation.
-   Participate in quality improvement initiatives and clinical team meetings.

## Requirements

-   MD or DO degree from an accredited medical school.
-   Board Certified or Board Eligible in Family Medicine.
-   Eligible for unrestricted Ohio medical licensure.
-   Strong diagnostic and clinical decision-making skills.
-   Excellent communication and interpersonal abilities.
-   Commitment to patient-centered, evidence-based care.
