## Description

**RN Home Health Care Manager**

**Full-time/Direct-Hire Position | Monday – Friday | 8 a.m. to 5 p.m.**

The RN Home Health Patient Care Manageris integral to delivering compassionate, high-quality home health services.

Through effective coordination, clear communication, and adherence to professional and regulatory standards, this leader drives a culture of excellence, safety, and trust — shaping quality outcomes and making a meaningful difference in the lives of patients and the professionals who serve them.

**Position Responsibilities:**

-   Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
-   Coordinate and monitor patient care delivery
-   Supervise clinical staff to ensure consistent, appropriate service
-   Maintain clear communication with patients, caregivers, referral sources, and field and office staff
-   Drive quality outcomes by meeting department goals and supporting improvement initiatives
-   Uphold compliance through required training, accurate reporting, and cooperation with audits

## Requirements

-   _**Registered nurse with current licensure to practice nursing.**_
-   _**Previous experience in home care setting with two years management or supervisory experience, preferred.**_
-   Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
