## Description

RN Hospice Clinical Liaison

We are currently hiring a full-time RN Hospice Clinical Liaison to join our team. This is a hospital-focused liaison role and not a traditional bedside nursing position.

**Position Responsibilities:**

-   Develop and maintain relationships with hospital referral sources and healthcare professionals.
-   Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
-   Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
-   Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
-   Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.

## Requirements

_**·   Active, unencumbered Registered Nurse (RN) license.**_

_**·   Hospital bedside nursing background, including medical-surgical (med-surg) experience.**_

·   Minimum 1 year of healthcare sales or marketing experience.

·   Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.

·   Valid driver’s license, auto insurance and dependable transportation.
