## Description

Are you a compassionate Physical Therapist who enjoys building meaningful relationships and helping patients improve their mobility and quality of life? Join our Home Health team in Loveland, where you'll provide one-on-one, personalized therapy that empowers patients to recover, regain independence, and safely remain in their own homes.

### What You'll Do

-   Perform comprehensive physical therapy evaluations in accordance with physician orders and agency guidelines.
-   Develop individualized treatment plans based on each patient's functional goals and clinical needs.
-   Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall function.
-   Educate and counsel patients and caregivers on treatment plans, home exercise programs, and injury prevention.
-   Communicate assessment findings and treatment recommendations with physicians and the interdisciplinary care team.
-   Monitor patient progress, adjust treatment plans as needed, and establish measurable rehabilitation goals.
-   Document evaluations, treatment sessions, progress notes, and care plans accurately and in a timely manner.
-   Collaborate with team members to ensure coordinated, high-quality patient care.
-   Maintain productivity standards while delivering exceptional clinical outcomes.

## Requirements

### What We're Looking For

-   Minimum of **9 months of experience** as a Physical Therapist.
-   Home health or community-based therapy experience preferred.
-   Master's degree in Physical Therapy from a **CAPTE-accredited** physical therapy program.
-   Current Physical Therapist license in the state of practice.
-   Current CPR certification.
-   Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service area.
-   Strong communication, organization, and patient education skills.
-   Passion for delivering compassionate, individualized care.
