Our home health census is growing, our therapy team is expanding, and we’re looking for talented **Physical Therapists (PTs) and Physical Therapist Assistants (PTAs)** who know how to make the magic happen in the home.

**💙 What You'll Do**

As a member of our therapy team, you’ll help patients regain strength, improve mobility, reduce pain, prevent falls, and return to the activities that matter most to them.

**PTs will:**

-   Complete comprehensive evaluations and identify functional needs.
-   Develop individualized plans of care and treatment goals.
-   Provide evidence-based interventions and monitor patient progress.
-   Reassess patients and modify treatment plans as clinically appropriate.

**PTs & PTAs will:**

-   Provide skilled therapy in accordance with physician orders and CMS Conditions of Participation.
-   Deliver therapeutic exercise, gait and balance training, transfer training, strengthening, and functional mobility interventions.
-   Educate patients and caregivers on fall prevention, assistive devices, safety, and Home Exercise Programs (HEPs).
-   Maintain timely and accurate documentation.
-   Collaborate with physicians, caregivers, and the interdisciplinary team to help ensure successful patient outcomes.

## Requirements

**💼 Our Ideal HHC Guru**

You may be our next **Home Health Magic Maker** if you have:

-   Degree from an accredited PT or PTA program.
-   Current Michigan PT or PTA license in good standing.
-   Home health experience preferred.
-   Strong clinical judgment and problem-solving skills.
-   Excellent communication and patient education skills.
-   Ability to work independently while remaining connected to the care team.
-   Reliable transportation, valid driver's license, and current auto insurance.
-   Familiarity with Medicare/CMS documentation preferred.
