About Total Life

Total Life is a telehealth company dedicated to older adults. We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.

We're launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy.

The role

You'll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You'll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.

What you'll do

-   Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
-   Provide medical follow-up visits for chronic disease management and ongoing care
-   Perform cognitive screening and staging; escalate for further assessment where indicated
-   Prescribe and manage GLP-1 therapy for appropriate patients
-   Conduct advance care planning conversations
-   Document in our EHR to support accurate Medicare billing

## Requirements

Requirements

-   Active, unrestricted NP license in New York
-   Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
-   Active DEA registration
-   Experience with Medicare documentation and E/M coding
-   Comfort with telehealth and EHR documentation
-   Reliable high-speed internet and a private, HIPAA-compliant workspace

Nice to have

-   Experience with comprehensive geriatric assessment
-   Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
-   GLP-1 prescribing experience
-   Multi-state licensure
