**Clinical consultancy, supervision and specialist escalation**

**Remote | Self-employed | Minimum 5 hours per week**

We are seeking an experienced Consultant Psychiatrist to provide clinical consultancy and supervisory support within our ADHD prescribing pathway. At Dr-Julian, we provide accessible mental health care via video, audio and messaging, and are expanding our neurodiversity services to improve access to care.

Job overview

Our model is senior prescriber led clinical supervision, with consultant psychiatrist oversight and specialist escalation. Experienced senior ADHD prescribers provide routine clinical support and first-line supervision to qualified independent ADHD nurse prescribers. You will supervise and advise those senior prescribers, contribute to clinical governance and provide specialist input on selected complex or higher-risk cases.

This is not a routine assessment or prescribing caseload role, and does not require automatic review or sign-off of every initiation, prescription or treatment review. Selected direct patient reviews or prescribing may be undertaken where clinically required, within your competence and agreed role arrangements.

Treating independent prescribers remain accountable for their prescribing decisions. You remain accountable for your own advice, supervision, clinical decisions and any care responsibilities you undertake. Dr-Julian retains responsibility for its service governance arrangements.

We offer

Regular access to CPD, a supportive multidisciplinary team and ongoing support from our Clinical Leadership team. Professional supervision and support arrangements appropriate to the role will be agreed at appointment.

Responsibilities

·   Provide scheduled clinical supervision and specialist psychiatric advice to designated senior ADHD prescribers and supervisors.

·   Advise on escalated cases involving diagnostic uncertainty, complex comorbidity, medication risks, adverse effects or treatment non-response.

·   Support safe initiation, titration, review, physical health monitoring and shared-care decisions through specialist advice and agreed pathway oversight.

·   Contribute to relevant clinical protocols, proportionate prescribing and case audits, incident learning and service improvement.

·   Record patient-specific advice, rationale and recommended actions in the clinical system, identifying responsibility for follow-up.

·   Participate in agreed escalation arrangements, allowing direct escalation when urgency, complexity or senior prescriber unavailability makes this necessary. Emergencies follow established urgent care and safeguarding pathways.

·   Work collaboratively with the wider team and relevant external services to support continuity, appropriate referral and safe care. Raise concerns about prescribing practice or clinical competence through the agreed governance process.

**Hours and availability**

Minimum commitment of 5 hours per week. Scheduled supervision, governance time and escalation availability will be agreed before starting. Response times, cover during absence and any additional clinical work and fees will be defined separately. This commitment does not imply continuous or out-of-hours on-call availability.

Essential qualifications and skills

·   Medical degree recognised by the GMC; full GMC registration with a current licence to practise; and entry on the GMC Specialist Register in a psychiatry specialty relevant to the agreed patient group.

·   MRCPsych or a recognised equivalent qualification, with substantial recent consultant-level experience in ADHD assessment, diagnosis and medication initiation, titration and review.

·   Experience of supervising independent prescribers, providing specialist consultancy and working within multidisciplinary services.

·   Competence in complex ADHD presentations, psychiatric comorbidity, risk assessment and appropriate escalation. Any work involving children and young people requires separately verified relevant competence.

·   Knowledge of NICE NG87, safe controlled drug prescribing, physical health monitoring and shared-care arrangements.

·   Current Level 3 safeguarding adults and safeguarding children training.

·   Appropriate insurance or professional indemnity covering the full agreed role, including remote consultancy, supervision of non-medical prescribers, relevant patient age groups and any direct clinical work.

·   Engagement with appraisal, revalidation and CPD, alongside strong communication, clinical documentation and digital working skills.
