**Position Overview:**

Greenlife Healthcare Staffing is seeking a dedicated **Licensed Practical Nurse (LPN)** to join a skilled care facility in Queens, NY. This role involves providing compassionate, high-quality care to long-term care residents, ensuring adherence to care plans, and collaborating with interdisciplinary teams to enhance patient outcomes.

**Key Responsibilities:**

-   Administer medications and treatments as prescribed.
-   Monitor and document patients’ vital signs and health status.
-   Collaborate with RNs, physicians, and therapists to implement care plans.
-   Assist residents with activities of daily living (ADLs) and mobility.
-   Ensure compliance with infection control and facility protocols.
-   Respond to emergencies and provide timely interventions.
-   Maintain accurate and timely medical records.

## Requirements

**Qualifications:**

-   **Education:** Completion of an accredited Licensed Practical Nursing (LPN) program.
-   **Licensure/Certifications:**

-   Active New York State LPN license.
-   Proof of Hepatitis B immunity (required).

-   **Experience**: Must have at least two (2) years of experience in a nursing home or hospital setting.
-   **Soft Skills**: Strong communication, adaptability to rotating shifts, and teamwork.
