Responsibilities

-   You’ll also have the structure and support you need to focus on what matters most: your patients
-   As a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care
-   You’ll work independently in the field - but never alone
-   Deliver skilled nursing care in accordance with the established plan of care
-   Monitor and assess patient conditions during home visits or shifts
-   Communicate changes in patient status to the Clinical Supervisor
-   Provide patient and caregiver education regarding medications, treatments, and disease management
-   Accurately document patient care and clinical observations
-   Coordinate care with other healthcare team members as appropriate
-   Maintain compliance with agency and state guidelines
