As a Social Worker with Veteran Pathways (part of the Proactive Health Group), you are the person who holds a client's care together from day one. You will run biopsychosocial assessments, take your seat in GP-led case conferences, shape the care plan, then walk alongside your client through a structured 90-day journey. **About the role**

As a Clinical Social Worker and Care Coordinator, you are the anchor of our Chronic Condition Management Program. You will own a caseload of veteran and Medicare clients and deliver everything by telehealth from a private, confidential setting. 

You will run intake assessments, deliver care plans and lead 90-day reviews. You will coordinate referrals across a full allied health panel, including mental health social work, physiotherapy, podiatry, exercise physiology, dietetics and occupational therapy. You will have real clinical autonomy that grows as you do, backed by a Team Leader who is there when you need them. Every day looks a little different, and every client is a whole person rather than a checkbox.  

**What we will support you to achieve**

-   Running comprehensive biopsychosocial assessments across physical health, mental health and social circumstances.
-   Coordinating care and navigating clients through the system as their go-to person. 

-   Presenting your findings and psychosocial recommendations in GP-led multidisciplinary case conferences.
-   Delivering care plans and leading 90-day reviews, then feeding updates back to the GP. 
-   Holding continuity through care coordination consults every two to three weeks. 
-   Screening for risk and escalating early when something needs it. 
-   Coordinating referrals and advocating for your clients across the team and the community. 
-   Documenting cleanly in Halaxy, in line with AASW, Medicare and DVA standards.

## Requirements

You are a social worker who leads with empathy and backs it with clinical judgement. You build rapport quickly, even down a phone or video line, and you are comfortable holding your own in a case conference. You like collaborating, you communicate clearly, and you care about coordinated, whole-person care. Telehealth does not faze you. If anything, you are good at it. 

-   Tertiary qualification in Social Work (Bachelor or Master) providing eligibility for AASW membership.
-   Current, unrestricted AASW membership, or eligibility to obtain it before commencement. 
-   Satisfactory National Police Check, provided prior to commencement. 
-   Disclosure of any relevant prior disciplinary or professional conduct history known to the AASW. 
-   A private, confidential space from which to deliver telehealth consultations.
