## Description

Our walk-in program provides rapid, accessible crisis intervention services for Ocean County adults experiencing mental health challenges that are impacting their day-to-day functioning. Services are recovery-oriented and time-limited, with a focus on helping individuals stabilize, identify next steps, and connect with longer-term behavioral health services and community supports. The program serves adults age 18 and older who can be safely supported in an outpatient setting and do not currently have other case management services in place.

The position provides comprehensive, community-based behavioral health services through assessment, treatment planning, crisis intervention, counseling, education, and recovery support. Working as part of a multidisciplinary team, this role helps consumers stabilize during periods of crisis, build skills, access community resources, and connect with ongoing care while promoting independence, dignity, and overall well-being.

_**Responsibilities:**_

-   **Clinical Assessment & Treatment Planning**

-   Complete comprehensive bio-psychosocial and ongoing clinical assessments.
-   Develop and update Comprehensive Treatment Plans, WRAP Plans, and/or IMR plans in collaboration with consumers.
-   Provide direct services in accordance with individualized treatment and recovery plans.
-   Monitor clinical status, emotional well-being, behavioral changes, symptoms, and medication compliance.
-   Engage consumers in treatment planning, service delivery, and community-based activities.
-   Maintain timely, accurate documentation of assessments, interventions, consumer responses, and care coordination.

-   **Multidisciplinary Care**

-   Participate in multidisciplinary treatment team meetings and contribute to coordinated consumer care.
-   Collaborate with treatment providers, FAST TRACK staff, consumer supports, and community partners as appropriate.

-   **Crisis & Recovery Services**

-   Provide crisis intervention, de-escalation, supportive counseling, and follow-up as needed.
-   Participate in assessment, treatment planning, and discharge processes related to crisis services.
-   Provide rotating 24-hour on-call coverage and communicate relevant contacts and outcomes to the treatment team.
-   Assess substance use needs and connect consumers with co-occurring services when clinically indicated.

-   **Life Skills & Community Support**

-   Assist consumers with accessing basic needs, including housing, benefits, transportation, and community resources.
-   Provide psychoeducation to consumers and families related to mental health, symptom management, crisis response, medication, and life skills.
-   Facilitate WRAP, IMR, and psychoeducational groups based on consumer and program needs.

## Requirements

-   Master's Degree in a Human Services field required
-   Experience with Crisis Intervention and outreach services preferred
