**About the Role:**  
We are seeking an Occupational Therapist (OT) to provide **in-home, individualized therapy** to **adult clients** in **Grand Junction, CO**. You’ll help patients improve independence, mobility, and overall quality of life through skilled, client-centered care.

**Compensation & Scheduling:**

-   **Competitive Pay:** $70.00 – $85.00 per routine visit
-   **Start of Care Visits:** Up to $120 per SOC
-   **Part-Time Availability:** 10–20 visits per week

**Key Responsibilities:**

-   Develop and implement personalized treatment plans
-   Conduct evaluations using standardized tools and clinical expertise
-   Provide therapy focused on ADLs, mobility, safety, and independence
-   Educate patients and caregivers on home programs and adaptive equipment
-   Maintain timely and accurate clinical documentation
-   Collaborate with a multidisciplinary care team to ensure coordinated support

## Requirements

-   A valid Occupational Therapist license in Colorado
-   Proven experience in home health or a related field
-   Excellent communication and interpersonal skills
-   Proficiency in electronic medical record documentation
-   A flexible and adaptive approach to meet the diverse needs of your clients
