**Position Overview:**

Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

**Key Responsibilities:**

-   Conduct utilization reviews for appropriate care level/coverage
-   Coordinate discharge planning and post-acute care transitions
-   Document care coordination in EPIC
-   Ensure compliance with payer guidelines and regulations
-   Collaborate with ED teams for rapid case management
-   Analyze resource utilization trends and recommend improvements

## Requirements

**Qualifications:**

-   **Education:** ADN/BSN from an accredited program
-   **Licensure:** Current and active New York State Registered Nurse (RN) license is required.
-   **Certification:** Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment.
-   **Experience:**

-   Must have Pediatric experience.
-   Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
-   Proven experience in hospital case management within an acute care environment is essential.

-   **Technical Skills:** Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.
