### **Physical Therapist (PT)  
Skilled Nursing Facilities | Multiple Locations Available (CA & AZ)**

**KEY DETAILS:**

-   13-Week Contracts
-   36–38 Hours per Week
-   $55–$67/hr.
-   Start ASAP

**Compensation**  
• $55–$67/hour (based on experience)  
• Weekly pay

**Your Role**  
• Perform patient evaluations and develop individualized treatment plans  
• Provide PT services to short-term rehab & long-term care residents  
• Improve patient mobility, strength, balance, and functional outcomes  
• Document progress and outcomes accurately  
• Collaborate with interdisciplinary care teams

**REQUIREMENTS**  
• Active California or Arizona PT license (dependent upon assignment state)  
• Prior experience in a Skilled Nursing Facility  
• Strong communication & documentation skills  
• Ready to start ASAP
