### Primary Care / Family Medicine Physicians

**Full-Time | Outpatient-Focused**

### Position Summary

A well-established medical practice is seeking a **full-time Primary Care / Family Medicine Physicians** to join a collaborative care team in a community-based outpatient setting. This opportunity is ideal for a physician who enjoys providing comprehensive care to patients of all ages and values continuity of care within a supportive practice environment.

### Responsibilities

-   Evaluate, diagnose, and treat patients using appropriate medical judgment and evidence-based practices
-   Provide comprehensive primary care services across the lifespan
-   Coordinate patient care, including referrals, follow-up, and care transitions
-   Participate in quality improvement initiatives to enhance patient outcomes and cost-effective care
-   Maintain accurate and timely clinical documentation that meets regulatory and payer requirements
-   Participate in inpatient and outpatient consultation and call coverage as needed
-   Engage in academic, educational, or research activities as requested

## Requirements

### Certifications & Licensure

-   **Board Certified or Board Eligible** in Family Medicine or Primary Care specialty
-   Active state medical license or eligibility to obtain licensure

### Required Education

-   Successful completion of an **ACGME-accredited residency program**
