### **Licensed Practical Nurse (LPN) – Weekend Private Duty (Friday 5 PM – Monday 8 AM)**

**KeystoneCare** is seeking a compassionate and dedicated **Licensed Practical Nurse (LPN)** to provide private duty nursing care for a patient with ALS in their home. This role is ideal for a nurse who is committed to delivering high-quality, patient-centered care while supporting both the patient and their family.

This position involves extended weekend coverage from **Friday at 5:00 PM through Monday at 8:00 AM**, providing continuity of care and ensuring the patient’s safety, comfort, and well-being.

**Primary Job Responsibilities:**

-   Provide skilled nursing care in accordance with physician orders, patient care plans, and agency policies
-   Administer medications and treatments as prescribed
-   Perform ongoing patient assessments and monitor for changes in condition
-   Ensure patient safety, comfort, and overall well-being throughout the shift
-   Communicate effectively with patients, families, and interdisciplinary care teams
-   Educate patients and families on care plans, equipment use, and safety measures
-   Maintain accurate and timely documentation in compliance with agency standards

## Requirements

**Job Requirements:**

-   Graduate of an accredited LPN program
-   Current LPN license in Pennsylvania
-   Minimum of one year experience in home healthcare or a similar clinical setting
-   Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift
-   CPR certification
-   Strong communication and interpersonal skills
-   Self-motivated with the ability to work independently
-   Reliable transportation and valid Driver's License
